The annual ICD-10-CM update brings hundreds of new, revised, and deleted codes that directly affect primary care. Staying current matters for accurate documentation, proper reimbursement, and audit readiness.

Key changes for primary care

Diabetes mellitus coding expansion

The diabetes category (E11) has expanded with greater specificity:

New subcategories for type 2 diabetes with specified complications now require more granular documentation
Diabetic foot ulcer codes now require location (left/right, specific toe/heel/midfoot)
Document diabetic retinopathy stage (mild NPDR, moderate NPDR, severe NPDR, PDR) for accurate E11.3xx selection

Hypertension coding updates

Essential hypertension (I10) is unchanged but should be confirmed as primary diagnosis
Secondary hypertension codes (I15) now include drug-induced and renal artery stenosis specificity
Document resistant hypertension when a patient requires 3+ medications at maximum doses
